My own illustrated version of the book's cover, painted on an actual copy of the book itself!

Process and materials: I've wrapped a used paperback copy of Wolf Hall by Hilary Mantel in a new book-cover made from colored cardstock; then I've re-drawn the front cover and spine using water-based paints, gel pens, and permanent marker, and treated it with a light invisible matte varnish for protection from scuffing. The back cover is blank red cardstock.

Display and use: I like to envision this displayed on a bookshelf, but you could also frame it in a deep shadow box. Additionally, it is a real, readable copy of the book, so you could read it if you wanted, though that would likely result in a bit of wear-and-tear on the art (I don't mind if you don't). Also, do be aware that these materials are not entirely lightfast, and could fade if displayed in direct sunlight over long periods of time.
